To enroll please follow these procedures:
1. Familiarize yourself with the Handbook

2. Fill out the Church School Enrollment Form (CSEF),
Family Information Form (FIF) and the Request for School Records (RFR)
if records are needed from previous school.

3. Send all 3 forms and $65 enrollment fee to our address.
I will sign and return forms to you.
You can send a check, money order or use
PayPal (vbidwell@charter.net)

When you receive signed forms:
1. Make a copy of the CSEF and keep for your records.
Send original to the county or city BOE.

2. Send RSR to the last school attended.

3. Let me know if the student has been expelled or suspended.
There may be additional requirements.

4. Also, let me know if you plan on returning student to public school
within the school year or the following school year.

Your child is considered enrolled once I have received the CSEF, FIF and fee.
It is the parents' responsibility to deliver or mail the CSEF and RSR form to
appropriate people.
